RELEASE OF LIABILITY ​

1. I, on behalf of myself and/or as parent/guardian of my minor child/children, individually and collectively) acknowledge that I/we have voluntarily agreed to participate in charitable activities sponsored by Shoes For The Homeless, Inc., on the premises at West Los Angeles College (WLAC), 9000 Overland Avenue, Lot 7, Culver City, CA 90230.
2. I/we hereby agree that I/we, and/or our representatives will not make a claim against, sue, or attach the property of WLAC or Shoes for the Homeless, Inc. for injury or damage resulting from negligence or other acts, howsoever caused. I/ we hereby release WLAC and Shoes for the Homeless, Inc. from all actions, claims, or demands that I/we, our assignees, heirs, distributees, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from our activity or use of the premises.
3. By signing below, you give your permission for Shoes for the Homeless, Inc. to publish group photographs that may include any or all participants in a variety of ways. The publications could include, but are not limited to, our website, emails or local newspapers. If published, third parties would be able to view the photographs.
KNOWING AND VOLUNTARY EXECUTION
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND SHOES FOR THE HOMELESS, INC. AND SIGN IT OF MY OWN FREE WILL.
